be completed to open a Commercial Charge
Account. Then, complete and return all
the required forms listed below. Please
note that a Commercial Charge Account
may be opened with either a Bond or Cash
Surety Deposit (see Section B, Surety
Forms, on page 2 of the Application Booklet
for more information).
